LEVELS OF CARE

Routine Care: Hospice Care is provided wherever the patient calls home, visits from the hospice team in your home; wherever you call home.

Crisis Care: 24 hour nursing care is provided in your home setting when a crisis arrives, in other words a symptom needs continuous nursing care to manage that symptom.

Continuous Care: provided on a short term basis. Inpatient care: patient care for symptom management/pain control that cannot be managed in the home setting. Once managed the patient is returned to where they call home.

Respite Care: Short term care, usually five days or less, that is provided in a skilled nursing setting. Respite care is provided to give family relief.
